Translating Civil Records?

Is there anyone who can either translate a couple of images from the Civil Registers off of FamilySearch for me?  I believe I've finally found my great-great grandfather's birth registration but do not know Dutch and am having a hard time reading the record.  I had some german in high school and can pick out a couple of things to know what they mean but would love to have a transcribed translated copy of the images.
